LASER CARTRIDGE REFUND

Do not discard your EMPTY LASER CARTRIDGES in the waste receptacle; help the campus
recycle by returning it to GENERAL STORES, MMDC for a $2.00 refund. These refunds will be processed on Journal Vouchers at periodic intervals. We would prefer the empty cartridge be returned in the manufacturer's carton along with the styrofoam packing and felt cleaner.

After completing this form, print two copies. Attach one to the outside of the cartridge box and keep one for your records.
